1. Download and print the Membership and Donation Mail In form.
2. Fill out the "Donor/Member Information" section of the form.
   • This form can be used to make a Donation, to sign up for membership, or both if you choose.
   • This section must be filled out even if you only wish to make a donation for tax receipt information
3. (Optional) If you wish to sign up or renew a membership, fill out the "Membership in the Conservative Party of Canada" section. If you only wish to donate skip this section.
4. (Optional) If you wish to donate fill out the "Nepean Conservative Association Donation Options" section.
5. Fill out the "Payment Details" section for your chosen method of payment.
6. Place completed form (and cheque if that is your method of payment) into an envelope and address it to 

   Nepean Conservative Association
   P.O. Box 89031, 900 Greenbank Road
   Nepean, ON K2J 1S0

7. Seal envelope, place postage, and send. Please allow 4 weeks for processing. Our volunteer team will process these once a month.
